PSD Physical Form if your provider does not have one of their own
A physical examination is required prior to the first practice for
participation in interscholastic athletics. According to a new ruling by
the Washington Interscholastic Activities Association (WIAA), most
physical exams are valid for 24 months rather than the previous 13
months.
The physical exam must stipulate that the athlete is cleared for
competition specifically at the junior high level — or for competition
at the high school level. For example, a physical exam obtained as a
junior high ninth grader within the past two years must indicate that
the athlete is also cleared for high school competition if the athlete
intends on participating as a tenth grader in his or her new high
school.
